Testosterone and prostate: enemies or friends?

Historically, testosterone has always been considered a dangerous hormone for the prostate. In fact, for a long time it was thought to be one of the culprits in the process behind diseases such as the’benign prostatic hyperplasia and the prostate cancer.

Prostate tissue is certainly responsive to the action of androgen hormones, but the role of these on the development and progression of prostate disease is still a matter of debate.

In contrast, hormone therapy is not useful for the treatment of early-stage neoplastic disease.

Boosting testosterone as prostate therapy

That androgens stimulate prostate cancer growth is a scientific fact. However, there is no clear data on whether high levels of endogenous testosterone can promote the development of this disease in men.

Similarly, it is unclear whether the administration of exogenous testosterone has risks in patients with prostate cancer.

Physiological testosterone production gradually declines with age, or may be impaired in men with hypogonadism.

Hypogonadism is a pathological situation in which androgen deficiency is associated with the development of various symptoms, including metabolic disorders, fatigue and loss of energy, decreased libido, erectile dysfunction, etc.

However, although testosterone replacement treatment can bring significant benefits to a man suffering from hypogonadism, concerns about potential prostate side effects have severely limited its use.

A meta-analysis of the scientific literature showed that less than 2% of prostate cancer patients operated on and treated with testosterone experienced biochemical recurrence (source).

The currently available studies are not yet numerous, but they certainly suggest that patients in active surveillance may benefit from hormone treatment for hypogonadism.

Based on this, the question therefore arises as to whether patients who have had their prostate removed can also receive testosterone treatment to treat erectile dysfunction, without increasing the risk of developing a relapse.
